Nurse Practitioner — House Calls

Provide primary, transitional, and chronic care to homebound patients across southeast Michigan. You’ll see roughly 6–9 patients per day on a route built by our schedule optimizer. We provide a fleet vehicle stipend, full mobile EMR, and a back-office team that handles intake, prior auth, and billing so you can focus on patients.

Responsibilities

  • Conduct comprehensive in-home medical evaluations and follow-up visits
  • Manage acute, chronic, and palliative conditions in collaboration with the medical director
  • Document care in real-time on a tablet using our EMR + voice dictation
  • Coordinate with home health, hospice, pharmacy, and specialty care partners
  • Support transitional care management within 48 hours of hospital discharge

Qualifications

  • Active Michigan NP license + DEA
  • FNP, AGPCNP, or AGACNP board certification
  • 2+ years of primary care, geriatrics, palliative, or hospital-medicine experience preferred
  • Comfortable driving 60–80 miles/day; reliable smartphone

RN Care Manager — Chronic Care Management

Lead our CCM/RPM program. You’ll own a panel of ~150–200 chronic-disease patients, run monthly care-plan calls, manage medication adherence, and triage red flags to the clinical team.

Responsibilities

  • Conduct monthly CCM check-ins and care-plan updates
  • Triage symptom changes and escalate to the on-call clinician
  • Document time and interventions for Medicare CCM/RPM billing
  • Coordinate with home health, hospice, pharmacy, social work

Qualifications

  • Active Michigan RN license, BSN preferred
  • 3+ years experience in case management, home health, or chronic disease management
  • Strong written communication; comfortable with EMR documentation

Medical Assistant — Field

Responsibilities

  • Vital signs, phlebotomy, EKGs, urinalysis, wound photography
  • Specimen handling and chain-of-custody to lab partners
  • Visit documentation, photo upload, EMR data entry
  • Inventory management of medical supplies in the field vehicle

Qualifications

  • CMA or RMA certification (or willing to obtain within 90 days)
  • Phlebotomy proficiency required
  • Valid driver’s license + clean driving record

Intake & Scheduling Coordinator

Qualifications

  • Detail-oriented and patient-focused communication style
